Enrollment Interest Application

Parents interested in enrolling their child(ren) in the WUSTL Family Learning Center, please download and complete the Pre-Enrollment Interest/Waiting List Application form.

Standard Forms

  • Missouri Child Care Enrollment Form* (found on BH Enrollment page)
  • Child Medical Examination Report* (found on BH Enrollment page & completed by child’s pediatrician, no parent signatures are allowed)
  • Immunization Records* (from pediatrician)
  • Child’s Information Form (found on BH Enrollment page)
  • Informed Consent Form* (found on BH Enrollment page)
  • Topical Ointment Administration (found on BH Enrollment page – required for any lip balms, lotions or diaper creams not prescriptions)
  • Medication Authorization Form (found on BH Enrollment page – required for any medication not covered by Topical Ointment Administration form, including prescriptions, over the counter and any medications given not in accordance with medications labeled instructions for dosage or use.  Doctors signature may be required).

Infant/Toddler Addendum (6 weeks to 2 years of age)

For children with medically diagnosed or suspected allergies, health care plans requiring life saving medication, and/or food preferences:

  • Allergy Health Care Plan* (found on BH Enrollment page) To be used for medically diagnosed allergies only, and requires pediatrician and parent/guardian signature 
  • Suspected Allergy/Food Intolerance Form (found on BH Enrollment page)
  • Food Preference Form (found on BH Enrollment page) To be used for cultural, religious, vegetarian or vegan
  • Asthma Health Care Plan*(found on BH Enrollment page)

Back-Up Care Advantage Program (BUCA)

Please visit the BUCA website for information for registering for BUCA, which also provides a link for registration materials when using BUCA. Requires a Username and Password provided by your employer.
